STEP 1: Complete the SFC Registration Application
The SFC Registration Application should be completed by the camper's parent or guardian. To complete the SFC Registration Application you will be required to submit payment for the registration fee ($45), weekly sessions ($199 each) or for all 8 sessions ($1492 - save $100). Payments can be submitted via PayPal (debit/credit card - no account necessary) or alternate payment options such as Venmo or money orders.
STEP 2: Download and complete a Medical History Form
You can email the completed form to firstname.lastname@example.org or submit in person.
